The role emphasizes culturally responsive services, housing facilitation for refugees, and collaboration with local partners to secure resources.

Qualifications

• Bachelor’s degree or equivalent experience in social services.
• 3–4 years of case management/direct service experience.
• Strong knowledge of trauma-informed, culturally responsive care.
• Excellent written and verbal communication and documentation skills.

Responsibilities

• Conduct intake assessments to determine client needs, risks, and eligibility.
• Develop individualized service plans with measurable goals and timelines.
• Provide ongoing case management and monitor progress.
• Facilitate access to housing, healthcare, employment, education, legal support, and financial assistance.
• Offer crisis intervention and safety planning as needed.
• Advocate for clients with external agencies to secure resources.
• Maintain relationships with community partners and service providers.
• Connect clients to internal and external programs aligned with goals.
