Job summary

The Salvation Army Southern California is seeking a Case Manager to provide professional, individualized case management services to assist participants in addressing homelessness and barriers to independence.

You will conduct psychosocial assessments, develop Housing & Services Plans, coach on job readiness, manage budgeting, and document progress in HMIS/Clarity. Collaboration with community partners and timely reporting are essential. Evening, weekend, and holiday hours may be required.

Qualifications

  • BA/BS preferred in social work or related field.
  • 1 year of work experience with special needs, low-income and/or homeless populations.
  • Current knowledge of and interest in homeless populations and available supportive resources.
  • If in recovery, must have 2 years of sobriety.
  • Understand and support the mission of The Salvation Army.
  • Must obtain CPR and First Aid certification prior to employment.

Responsibilities

  • Meet with each participant weekly to assess and support goals and provide ongoing trauma-informed care.
  • Develop Housing & Services Plan (HSP) and Monthly Action Plan (MAP) for each client.
  • Strengthen job readiness through coaching in job search, resumes, and interviewing.
  • Assist clients in budgeting and saving, tracking income and savings.
  • Complete assessments and coordinate with CES and housing resources.
  • Maintain client data in HMIS/Clarity and paper files with timely notes.
  • Submit program reports and coordinate with partner agencies.
  • Collaborate with VA Liaisons, VASH, SSFV, HVRP, MHP, CBEST, legal services, transportation, health services, and housing providers.
  • Deescalate and investigate incidents and grievances; file reports if needed.
  • Attend trainings and meetings as required by funders and management.
  • Provide life skills classes and participate in audits to ensure data accuracy.
  • Work beyond normal hours and in other positions as needed.
