Job summary

The Salvation Army USA Western Territory in Bell, CA seeks a Case Manager to provide professional, individualized case management, information, and referrals to help participants address homelessness and gain independence.

You will conduct assessments, develop service plans, support job readiness, manage data in HMIS, and collaborate with agencies to connect Veterans and others with housing resources.

Qualifications

• BA/BS in social work or related field with 1 year of experience with special needs, low-income or homeless populations.
• Knowledge of homeless populations and available supportive resources.
• If in recovery, must have 2 years of sobriety.
• Must obtain CPR and First Aid before employment.
• TB clearance prior to hire.
• Understand and support The Salvation Army mission.

Responsibilities

• Provide case management and referrals to address underlying causes of homelessness.
• Meet participants weekly to assess progress and guide toward housing.
• Develop ISP and monthly action plan with clients.
• Strengthen job readiness through coaching and resumes.
• Maintain client data in HMIS/Clarity and paper files within 72 hours.
• Collaborate with VA Liaisons, VASH, HUD resources to link housing.
• Provide life skills training and assist with audits.
