Job summary

The Salvation Army USA Western Territory is seeking a Case Manager to provide individualized support to participants addressing homelessness and barriers to independence. The role emphasizes trauma-informed care, housing first principles, and a comprehensive ISP-based plan.

You will work with clients to assess needs, coordinate services, and track outcomes, including housing placement efforts, budget planning, and employment readiness while collaborating with community partners and funders.

Qualifications

• BA/BS degree in social work or related field required with 1 year experience with special needs, low-income and/or homeless populations.
• TB clearance prior to hire.
• CPR and First Aid certification required prior to employment.

Responsibilities

• Meet with participants weekly to assess progress and provide motivation.
• Develop ISP and monthly action plan with clients for housing goals.
• Coach clients on job readiness, resumes and interview skills.
• Assist clients with budgeting and income/savings plans.
• Complete VISPDAT/Next Step and connect to housing resources.
• Maintain HMIS/Clarity data and complete case notes within 72 hours.
• Submit program reports and collaborate with partner agencies.
• Deescalate incidents and handle grievances as needed.
• Attend trainings and provide life skills classes as directed.
