Job summary

The Salvation Army USA Western Territory is seeking a Case Manager to provide professional, individualized case management and referrals to assist participants addressing homelessness. You will develop housing plans, coordinate services, and support residents using Trauma-Informed care and Housing First models.

Qualifications

• BA/BS degree preferred in social work or related field with 1 year of relevant experience.
• Or a high school diploma with 2-3 years direct mental health or intensive case management experience.
• Current knowledge of homeless populations and available supportive resources.
• If in recovery, must have 2 years of sobriety.
• Understand and support the mission of The Salvation Army.
• Must obtain CPR and First Aid certification prior to employment.
• TB-cleared prior to hire.

Responsibilities

• Meet with participants weekly to assess goals and provide ongoing support.
• Develop a Housing & Services Plan and Monthly Action Plan with each participant.
• Strengthen job readiness skills through coaching in job search, resumes and interviews.
• Assist clients with budgeting and saving plans; track income/savings when applicable.
• Complete assessments and coordinate access to CES/HMIS resources with partners.
• Maintain HMIS/Clarity data and complete notes within 72 hours of meetings.
• Submit program reports; collaborate with housing resources and social service agencies.
• Attend trainings; provide life skills classes; participate in audits.
