Job summary

Colorado Coalition for the Homeless seeks a Case Manager I to work with families and individuals experiencing homelessness. You will develop care plans, coordinate services, and advocate for clients across health and housing resources.

You will monitor progress, document outcomes in the EHR, and collaborate with a team to ensure safe, trauma-informed support and referrals. Must have strong communication and community engagement skills.

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, or Criminal Justice preferred.
• Basic Life Support Certification required within 30 days of employment.
• Colorado driver’s license within 30 days of hire.

Responsibilities

• Manages a caseload of clients with high-service utilization and disabilities.
• Coordinate services and resources across community partners and health providers.
• Develop and implement care plans; monitor client progress and outcomes.
• Provide advocacy, support, and crisis intervention as needed.
• Maintain timely, accurate case documentation in the EHR.
