Job summary

CCH is seeking a Case Manager I to collaborate with case managers, supervisors, and community partners to promote long-term solutions for individuals experiencing homelessness or who have experienced homelessness. You will develop care plans, coordinate services, monitor progress, and advocate for clients in crisis situations.

The role focuses on outreach, referrals, and linkage to housing resources, with an emphasis on trauma-informed, strengths-based, and recovery-focused approaches.

Qualifications

• Bachelor's degree in Social Work, Psychology, or related field.
• Experience with homeless services or community-based programs preferred.
• Basic Life Support certification required within 30 days.
• Colorado driver’s license required within 30 days of hire.

Responsibilities

• Manage a caseload of clients with high-service utilization and disabilities.
• Develop and implement wrap-around care plans and crisis support.
• Coordinate referrals with community partners and housing resources.
• Document data in the EHR and maintain up-to-date charts.
• Provide crisis de-escalation and advocacy as needed.
• Attend trainings to bring updates to the team.
