Job summary

Colorado Coalition for the Homeless seeks a Case Manager I to partner with clients experiencing homelessness or at risk. You will develop care plans, coordinate services and maintain case notes while delivering outreach and crisis intervention across the central Denver region.

Required: High School Diploma or GED, 1–2 years of relevant experience, and a valid Colorado driver’s license. Bilingual candidates are preferred for better client engagement.

Qualifications

• High School Diploma or GED required; Bachelor’s degree preferred in related field.
• 1–2 years of experience with diverse populations, especially mental health or homelessness.
• Colorado driver’s license required within 30 days of hire.
• Bilingual or multilingual skills preferred.

Responsibilities

• Manage a caseload of clients with high service needs, including mental health and housing-related services.
• Coordinate services with partners, ensure timely referrals, and document progress in the EHR.
• Provide crisis intervention and support using a wrap-around care management approach.
• Collaborate with community partners to promote safety and connect clients to benefits.
• Travel within the Denver-Metro area to meet clients and conduct outreach.
