Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, or Criminal Justice preferred.
• Basic Life Support Certification within 30 days of employment.
• Colorado driver’s license required within 30 days of hire.

Responsibilities

• Manages a caseload of clients with high-service utilization and disabilities.
• Maintain positive relationships with community partners and health providers.
• Support clients with wrap-around care management and crisis engagement.
• Liaise between internal program management, client referrals, and partners.
• Respond to crises with de-escalation and referrals.
• Develop and implement crisis and safety plans tailored to client needs.
• Track client data, engagement, and outcomes.
• Assist clients in applying for benefits (Social Security, Medicaid, food assistance).
• Coordinate referrals to external agencies and services as needed.
• Ensure timely recording of clinical/non-clinical services in the EHR.
