Job summary

Frontier Behavioral Health in Spokane offers a Case Manager role focused on outreach to adults experiencing homelessness and chronic mental illness. You will engage clients in shelters, transit hubs, and community sites, coordinating with mental health services and housing resources to support recovery.

The position demands collaboration with providers, completion of documentation, and travel between sites.

Qualifications

• Bachelor's degree in behavioral sciences with 2–5 years related experience with psychiatrically impaired populations.
• Master's degree in the behavioral sciences with 1 year experience with psychiatrically impaired populations.
• Must submit for Agency Affiliated Counselor Registration within 30 days of hire.
• Must have an unrestricted and valid driver’s license, be able to pass motor vehicle screening and driving test, have active car insurance, and be willing to use personal vehicle in work.

Responsibilities

• Provide outreach and engagement services to persons with chronic mental illness who are homeless.
• Provide consultation and training to homeless providers.
• Provide coordination and appropriate linkages to community resources and treatment agencies.
• Provide stabilization and transition services to individuals with a mental illness who are homeless.
• Coordinate with various mental health and chemical dependency treatment agencies and the criminal justice system.
• Represent the agency in a positive and professional manner.
• Complete required documentation and data entry, including CMIS if required.
• Participate in community meetings and supervision.
