Job summary

Aviva Family and Children’s Services is seeking a Wallis House Case Manager in Los Angeles, CA to provide on-site case management for residents, coordinate with internal teams and external agencies, and support housing goals for families. Qualifications

• Eligible to work in the United States with legal status.
• High School diploma or equivalent.
• Valid CA driver’s license and ability to pass background checks.
• Doj fingerprint clearance and health screening requirements.
• Strong organizational and communication skills for multi-tasking.
• Knowledge of social services systems and community resources.
• Basic data entry and computer literacy.

Responsibilities

• Provide targeted case management for resident adults and children.
• Create and monitor Individualized Service Plans (ISP).
• Coordinate with HMIS/EXYM/LAHSA and referring agencies.
• Support housing goals, income stabilization, and social supports.
• Collaborate with multidisciplinary team and Housing Navigator.
• Maintain confidentiality and comply with mandated reporting.
