Job summary

Catholic Charities in San Antonio seeks a Case Manager to deliver day-to-day and long-term case management for Guadalupe Home residents, coordinating services in home and community settings. You will assess needs, develop service plans, advocate for residents, and work with community resources, maintaining records to track progress toward safety and well-being.

The role requires trauma-informed care and sensitivity to diverse cultural contexts.

Qualifications

• Bachelor’s degree in social work, behavioral sciences or related field.
• 1+ year volunteer or paid case management experience with vulnerable populations in child welfare or nonprofit setting.
• 3 years’ case management experience may substitute for Bachelor’s degree.
• Bilingual English/Spanish preferred.

Responsibilities

• Assess needs of residents and develop comprehensive service plans.
• Coordinate with community resources and providers to meet needs.
• Input data and maintain records in agency software.
• Attend meetings, trainings, and court hearings as needed.
• Advocate for residents and monitor progress toward goals.
