Job summary

Archdiocese of San Antonio seeks a Case Manager to implement day-to-day and long-term case management for Guadalupe Home residents, both in the home and in the community. You will assess needs, develop service plans, and coordinate with community resources to meet individualized goals.

The role requires documenting progress, maintaining records, and ensuring confidentiality. Travel to appointments may be required, with a focus on culturally competent, trauma-informed care and strong team

Qualifications

• Bachelor’s degree in social work, behavioral sciences or related field.
• Minimum of 1-year volunteer or paid experience as a case manager with vulnerable populations.
• Minimum of 3 years’ case management experience may substitute for a Bachelor’s degree.
• Must be at least 21 years old.
• Valid driver license with clean driving record and vehicle liability insurance.
• Bilingual English/Spanish preferred.

Responsibilities

• Implement day-to-day and long-term case management for Guadalupe Home residents.
• Assess residents’ needs and develop comprehensive service plans.
• Coordinate with community resources and providers to meet needs.
• Monitor, evaluate and advocate for services to meet goals.
• Maintain accurate records, data entry, and reporting.
• Attend meetings and drive residents to appointments when needed.
• Ensure confidentiality and compliance with agency policies.
