Job summary

Good Shepherd Services, Ltd is seeking a Case Manager (Credible Messenger) to provide trauma-informed, youth-focused case management for 16–25 year olds at risk of violence. You will build trust, coordinate services, and supervise sustainable growth plans under a restorative framework.

Qualifications

• High school diploma or GED required with six years in related roles.
• Bachelor’s degree preferred in Social Work, Human Services, Psychology, or related field.
• Experience working with justice-involved or high-risk youth is preferred.

Responsibilities

• Maintain a caseload of approximately 7–8 participants.
• Conduct trauma-informed intake assessments using ARC screening tool.
• Develop and update Sustainable Growth Plans based on strengths, needs, and goals.
• Conduct weekly case management sessions and engage participants through office, home, hospital, and community visits.
• Build trusting relationships through mentorship, advocacy, and participant-centered support.
• Engage families and natural supports when appropriate.
• Coordinate referrals to behavioral health, housing, healthcare, and other services.
• Enter participant information into Salesforce and other databases; maintain confidentiality.
• Travel throughout NYC with potential evening/weekend hours.
