Job summary

StepStone Family & Youth Services seeks a compassionate Youth Services Case Manager to support children and families in foster care. You will coordinate services, advocate for clients, and provide individualized support to promote safety, stability, and positive outcomes.

The role involves collaboration with treatment teams, schools, referral agencies, and community providers to monitor progress, coordinate case plans, and ensure youth receive necessary services.

Qualifications

• Bachelor's degree in a human services field or nursing field.
• Two years of case management experience.
• Valid driver's license; strong verbal and written communication.

Responsibilities

• Receives and responds to referrals, identifies client needs, and consults with Director of Clinical Management to determine feasibility.
• Completes referrals with pre-screening assessments, reports, and case proposals; coordinates authorization and treatment team availability.
• Oversees field staff assisting with pre-screening.
• Accesses account information including contracts and payer terms as appropriate.
• Consults with third party representatives on benefit coverage, financial responsibility, and reimbursement procedures; negotiates levels.
• Contacts referral sources to inform case acceptance and introduce the clinical team.
• Maintains knowledge of services provided by the company and community resources; identifies alternatives when needed.
• Maintains relationships with referral sources and payer case managers; records outcomes.
• Monitors referral source satisfaction, tracks conversions, and provides reports to management.
• Implements processes to ensure compliance with policies and regulatory mandates; participates in HIPAA and ethics programs.
• Participates in growth planning and special projects; performs other duties as assigned.
