CHS in Rapid City is seeking a High-Risk Youth Case Manager to provide intensive case management, outreach, advocacy, service coordination, and trauma-informed intervention for youth and families affected by violence and trauma.

The role emphasizes a culturally responsive, strengths-based approach, collaboration with schools, behavioral health providers, and community partners to reduce risk factors and promote resilience in vulnerable youth.

Aufgaben

• Build rapport and develop therapeutic relationships with youth and families to assess needs, stabilize crises, intervene appropriately, and advocate for services that address immediate concerns and promote long-term success.
• Provide trauma-informed case management, advocacy, and support services to youth and families impacted by violence, trauma, behavioral health concerns, truancy, justice system involvement, and other significant life challenges.
• Conduct proactive outreach to youth exposed to community or household trauma and engage youth identified as high-risk through project referral pathways.
