Job summary

Community Care Alliance is seeking a dedicated Case Manager for our Youth Center in Woonsocket, RI. Join a team focused on delivering trauma-informed services to youth ages 12-25 within a drop-in center setting.

You will lead case management, direct services, and collaborative efforts with supervisors and local partners to support program success under WIOA guidelines. Responsibilities include data management, presentations to community groups, and developing engaging, leadership-focused

Qualifications

• Minimum of an Associate degree with relevant work experience.
• Bachelor’s degree in education, human services, or related field preferred.
• Must have demonstrated case management skills and experience with data management.

Responsibilities

• Data Support: Create, maintain, and update client files in a timely and accurate manner; collaborate with a multidisciplinary team to meet benchmarks; conduct presentations for community groups; design and facilitate programming for youth.
• Case Management: Engage with youth daily; participate in wraparound and family team meetings; recruit and retain participants; share opportunities with youth; provide direct services including intake, assessment, case management, and follow-up.
• Crisis intervention and de-escalation: Support staff in crisis situations; screen youth for safety and connect to emergency services; support families to coordinate with mental health providers; deliver trauma-informed wrap-around interventions.
