Job summary

Rooted Life is hiring a Community Support Specialist to provide person-centered support during daytime hours, executing care plans, helping with daily living activities, and coordinating with the Care Team. The role emphasizes safety, boundaries, and documentation within a supportive housing setting.

Training includes MHFA, Trauma-Informed Care, and behavioral crisis recognition. The opportunity offers a competitive target salary range and benefits in a fast-paced, mission-driven environment

Qualifications

• High School Diploma or GED required.
• Experience in residential, care, or advocacy settings preferred.
• Ability to maintain professional boundaries with clients.
• Comfort with hands-on daily living support and facility activities.
• Alert and able to stay engaged throughout shifts.
• Strong observation, communication, and problem-solving skills.
• Proficiency with Google Workspace and EHR systems.
• CPR/First Aid certification or willingness to obtain.
• Bilingual skills helpful (Spanish) but not required.
• Background check and drug screening required.

Responsibilities

• Conduct welfare checks and regular check-ins to ensure safety.
• Assist with activities of daily living and program participation.
• Support appointments, reminders, and transportation for clients.
• Coordinate with Care Team on schedules and follow-ups.
• Execute care plan activities and document progress.
• Maintain professional boundaries and redirect requests appropriately.
• Enforce house rules with warmth and accountability.
• Use motivational interviewing and de-escalation techniques.
• Observe and document changes; escalate to clinical staff as needed.
• Support implementation of treatment and behavioral health plans.
• Maintain accurate documentation in EHR and other systems.
• Participate in shift overlaps to ensure continuity of care.
• Maintain a safe, clean living environment; report safety concerns.
• Complete facility rounds and safety checks.
• Collect documentation from residents returning from hospitals.
• Report odors or safety hazards promptly.
• Uphold confidentiality and comply with policies.
• Attend trainings including Mental Health First Aid and Trauma-Informed Care.
• Maintain a safe, therapeutic environment.
