Job summary

The People Concern is seeking a dedicated Case Manager to support individuals experiencing homelessness with comprehensive, client-centered services. You will assess needs, develop service plans, connect clients to resources, and promote stability and well-being in a trauma-informed environment.

You will maintain housing plans, advocate for client needs, facilitate intakes for interim housing, and document progress in HMIS.

Qualifications

• Minimum of one (1) year of experience in case management.
• Experience with homelessness or at-risk populations.
• Ability to conduct crisis interventions non-violently.
• Ability to work independently in a fast-paced environment.

Responsibilities

• Provide active case management for a caseload of about 25 clients.
• Lead housing-related case management and maintain housing plans for clients.
• Connect clients with resources and navigate services to remove housing barriers.
• Build client relationships using motivational interviewing and trauma-informed care.
• Document case notes and progress in HMIS per funder requirements.
• Collaborate with a multidisciplinary team and participate in case conferences.
