Qualifications

• Three years of experience in care coordination, case management, or a related human services field.
• Prior supervisory or team lead experience preferred.
• Demonstrated experience with trauma, family violence, or complex social needs.
• Experience in healthcare, child welfare, domestic violence, or community-based settings.

Responsibilities

• Provide direct supervision to Care Coordinators and Care Navigators ensuring alignment with TCFSH protocols.
• Coordinate services including referrals, follow-up, and documentation.
• Provide onboarding and ongoing staff training on trauma-informed care and safety planning.
• Support staff in addressing high-risk and crisis situations.
• Track program metrics and contribute to quality improvement.
• Support implementation of organizational initiatives.
• Identify trends and opportunities to improve access and service delivery.
