Job summary

Sheboygan County Health & Human Services seeks a qualified professional to plan, provide, and monitor services for adults and youth in CCS, CST, and CLTS programs under supervision.

The role requires a bachelor’s degree in behavioral science or related field with extensive supervised post-degree experience; trauma-informed care, confidentiality, and collaboration with community partners are essential. Travel within the county is expected.

Qualifications

• Bachelor’s degree in behavioral science with 1,000 hours supervised post-degree clinical experience serving individuals with severe and persistent mental illness.
• Bachelor’s degree in a field other than behavioral science with 2,000 hours of supervised post-degree clinical experience serving persons with severe and persistent mental illness preferred.
• Knowledge of trauma-informed principles and practices is preferred.
• Knowledge of federal laws, State statutes, and administrative rules governing services.

Responsibilities

• Plan, provide, arrange, and monitor services for CCS, CST, and CLTS programs under supervision.
• Develop and implement treatment plans and crisis plans using trauma-informed, person-centered approaches.
• Maintain timely documentation including assessments and treatment plans.
• Coordinate with healthcare providers and community agencies to ensure continuity of care.
• Assist families with transitions to adult services and connect to resources.
