Job summary

Nationwide Children's Hospital is seeking a supervisor for Care Coordinators who support children, adults, and families facing abuse, trauma and complex psychosocial needs. You will lead care navigation, ensure seamless coordination, and align workflows with hospital protocols across teams.

You will train staff in trauma-informed care, manage referrals, and guide performance improvement while collaborating with clinical, legal, research, and community partners.

Qualifications

• Bachelor's degree in Social Work, Counselling, Public Health, or related field required.
• Master's degree preferred.
• Three years of experience in care coordination or related field; supervisory experience preferred.

Responsibilities

• Supervise Care Coordinators and Care Navigators following TCFSH protocols.
• Coordinate services, referrals, follow-up, and documentation timely.
• Provide onboarding and ongoing trauma-informed staff training.
• Support staff in high-risk and crisis situations with resource navigation.
• Track program metrics and contribute to quality improvement.
