Job summary

Health Connect America seeks a Therapist II to deliver high-quality mental health services to children, families, and adults. You will perform biopsychosocial assessments, develop individualized treatment plans, and maintain thorough documentation while collaborating with a multidisciplinary team.

Role includes outreach to promote services, crisis intervention when needed, and adherence to regulatory standards.

Qualifications

• Must hold one of the Florida credentials: RMHCI, LPC, LMFT or LCSW.
• Minimum of one year experience working with children and/or families.

Responsibilities

• Provide high-quality biopsychosocial assessments and determine needs.
• Develop and implement individualized treatment plans.
• Maintain accurate documentation and timely progress notes.
• Collaborate with multidisciplinary team and participate in meetings.
• Engage in outreach to promote services and attract referrals.
