You will conduct assessments, develop treatment plans, and collaborate with a multidisciplinary team to deliver care at home, in communities, or in office settings.

The role emphasizes client-centered care with a focus on healing and growth, ongoing training, and compliance with regulatory standards.

Qualifications

• Master’s Degree in Social Work, Counseling, or a related field.
• Must be eligible for licensure as a LPC, LMFT, or LCSW.

Responsibilities

• Perform biopsychosocial assessments to determine needs and interventions.
• Create individualized treatment plans with individuals, families, and the treatment team.
• Deliver individual, group, and family therapy sessions.
• Maintain accurate documentation and progress notes within policy timelines.
• Engage in outreach and community promotion of services.
