Job summary

Orlando Regional Medical Center, part of Orlando Health, is seeking an Aftercare Coordinator to develop, coordinate, and evaluate post-discharge recovery and survivorship services for trauma and burn patients within our Level I Trauma Center and ABA Verified Burn Center.

You will lead survivorship initiatives, collaborate with inpatient and outpatient teams, peers, and community resources to ensure seamless care, improve psychosocial well-being, and help families navigate recovery.

Qualifications

• Associate degree or higher in nursing or health care field or 8 years healthcare experience.
• Minimum two years clinical experience in rehab, social services, or care coordination.
• Strong data handling and confidentiality practices (PHI).
• MS Word and Excel proficiency.

Responsibilities

• Coordinate Trauma Survivors Network (TSN), SOAR, Journey Forward, peer support, and survivorship programs.
• Conduct post-discharge outreach and provide ongoing support to trauma and burn survivors and their families.
• Recruit, train, and support peer volunteers and survivor mentors.
• Serve as liaison between inpatient trauma, outpatient clinics, rehabilitation providers, and community partners.
• Monitor patient engagement, program participation, and recovery outcomes.
• Maintain program records, reports, and performance metrics.
• Assist with documentation for accreditation and regulatory bodies as assigned.
