Job summary

Orlando Health in Orlando, FL is seeking an RN Care Coordinator for the Medical Group's Trauma and Burn Clinic. The role collaborates with the assigned clinical team to identify patients who will benefit from care coordination, assessing risk factors and determining need for clinical utilization management and preventive services.

Full-time, 40 hours/week, Monday–Friday 8am–5pm. BSN required, FL RN license and BLS certification; three years of direct clinical experience.

Qualifications

• BSN required for new hires and transfers.
• Current Florida RN license and BLS certification.
• Three years of direct clinical experience in acute or ambulatory care preferred.

Responsibilities

• Assess patients’ needs and develop individualized care plans.
• Collaborate with the clinical team to coordinate care and manage risk factors.
• Support preventive care services and utilization management as needed.
• Provide patient education and safety guidance throughout care coordination.
