Job summary

Qualifications

• Active California RN license.
• Bachelor’s degree in Nursing (BSN) required; Master’s preferred.
• Excellent communication and interpersonal skills.
• Ability to coordinate complex transplant care across multidisciplinary teams.

Responsibilities

• Coordinate outpatient transplant care including clinic visits and test reviews.
• Maintain UNOS waitlist and donor/recipient communications.
• Develop policies and procedures for transplant program.
• Provide education to staff, patients, and families on transplant evaluation and listing processes.
• Document compliance with CMS/OPTN/UNOS regulations and clinical education.
• Act as a resource to staff for transplant patient care and family support.
• Oversee staff assignments and supervise completion of tasks.
• Collaborate with care teams across departments and participate in QAPI activities.
