Job summary

The University of Virginia is seeking a highly skilled RN to serve as Transplant Coordinator. You will work within a collaborative multidisciplinary team to deliver relationship-based care, assess and plan care, and educate patients across all transitions of the transplant continuum.

You will provide afterhours/weekend coverage as the primary contact for pre- and post-transplant patients and referring providers, coordinating complex care and ensuring timely communication among the team.

Qualifications

• Bachelor of Science in Nursing required.
• 2 years of relevant experience preferred.
• Licensed to Practice as a Registered Nurse in Virginia; BLS certification required.

Responsibilities

• Provide afterhours/weekend coverage as primary point of contact for pre- and post-transplant patients and providers.
• Conduct telephone assessments with strong clinical knowledge and coordinate complex care.
• Educate patients, families, and staff about transplant processes, risks, and care plans.
• Ensure multidisciplinary team members are informed of clinical decisions and patient responses.
