Job summary

Mayo Clinic is seeking a Transplant Coordinator Nurse to manage care coordination across the transplant patient population. You will collaborate with patients, families, and a multidisciplinary team to plan, educate, and implement care throughout the continuum.

Responsibilities include data interpretation for age-specific needs, leadership in quality initiatives, and adherence to ANA/UNOS/FACT guidelines. Requires RN licensure and transplant coordination credentials within three years of hire.

Qualifications

• Graduate of a baccalaureate nursing program.
• Minimum 1 year of clinical RN experience preferred.
• Must be able to work flexible hours and adapt to unpredictable situations within the patient care setting.
• Demonstrated leadership ability and interpersonal effectiveness required.
• Ability to work both autonomously and in a team environment.
• Strong communication, critical thinking, problem solving, and computer skills required.
• Current RN license by applicable state requirements.
• Maintains Basic Life Support (BLS) competency.
• Coordinators in Transplant must obtain either Certified Clinical Transplant Coordinator (CCTC) or Certified Clinical Transplant Nurse (CCTN) credential within three years of hire.
• Recertification must be maintained for the duration of the employee’s tenure in the transplant position. Additional specialty certification/training may be required by work area.

Responsibilities

• Coordinate all aspects of care for the transplant patient population.
• Assess and plan in collaboration with the patient, caregiver, and multidisciplinary healthcare team to meet needs across the care continuum.
• Assess data reflective of the patient’s status and interpret information relative to age-specific needs.
• Provide patient and family education on a consistent basis.
• Provide leadership through community involvement, committee work, and participation in quality improvement and research.
• Follow ANA Nursing: Scope and Standards of Practice, the ANA Code of Ethics, UNOS, and FACT guidelines as the basis for practice.
