Job summary

S:US in New York is seeking a Case Manager to support families facing housing instability. You will conduct intake within 24 hours, assess psychosocial and housing needs, and develop service plans with family participation.

You will coordinate with housing providers, accompany families to appointments, advocate for clients, and help secure permanent affordable housing while maintaining documentation and on-call crisis support.

Qualifications

• Bachelor's Degree in Human Services or related field is required.
• MSW or CASAC credentials are desired.
• Experience working with chronically homeless individuals with co-occurring disorders preferred.

Responsibilities

• Conduct intake within 24 hours of admission, explaining rights and grievance procedures.
• Assess psychosocial, vocational/educational, and housing needs and document findings.
• Develop and monitor individual and family service plans with family members.
• Coordinate with housing providers, real estate contacts, and landlords to secure housing.
• Advocate for families, participate in crisis intervention, and support daily living skills.
