Job summary

Priority Dispatch Corp. in the New York City area seeks a Social Worker to provide case management, crisis intervention, and resource coordination for clients in a Transitional Housing Program, using trauma-informed and person-centered approaches to support housing stability and recovery.

The role involves collaboration with an interdisciplinary team, development of service plans, advocacy for housing and community resources, and ensuring HIPAA compliance, with a 35-hour work week and full-time

Qualifications

  • Bachelor of Social Work degree is required.
  • Minimum of one year of experience providing case management, crisis de-escalation, or supportive services to individuals with mental health issues.
  • Experience with vulnerable populations in supportive housing, transitional housing, residential treatment, homeless services, or community-based behavioral health settings is preferred.
  • Experience conducting non-clinical intakes, developing individualized service plans, and coordinating care with multidisciplinary external teams.
  • Bilingual proficiency is strongly preferred.

Responsibilities

  • Conduct comprehensive, non-clinical biopsychosocial intakes, resource assessments, and housing readiness screenings for clients.
  • Complete functional inventories, baseline safety screenings, and ongoing program reviews as required.
  • Develop individualized service plans and housing goals in collaboration with clients.
  • Provide individual supportive guidance, crisis de-escalation, psychoeducation, and recovery-focused skills training.
  • Utilize evidence-based, non-clinical interventions based on Motivational Interviewing, Harm Reduction strategies, and Trauma-Informed Care principles.
  • Assist clients with identifying and addressing their housing, employment, family, legal, medical, and/or social support needs.
  • Identify clients requiring higher levels of external care and coordinate appropriate community referrals.
  • Coordinate care and maintain communication with external psychiatrists, psychiatric nurse practitioners, therapists, primary care providers, hospitals, and community-based organizations.
  • Assist clients with obtaining public benefits, healthcare services, supportive housing, employment services, and other community resources.
  • Advocate on behalf of residents to ensure access to appropriate services and community support.
  • Facilitate successful transitions to permanent housing and ongoing community behavioral health treatment.
  • Develop and maintain collaborative relationships with external social service providers and referral sources.
  • Evaluate urgent facility situations and implement appropriate non-clinical de-escalation interventions to maintain resident safety and program stability.
  • Conduct safety planning and wellness reviews when indicated.
  • Support clients through practical guidance during behavioral or substance use-related crises.
  • Coordinate emergency interventions with mobile crisis teams, hospitals, emergency services, and external behavioral health providers as necessary.
  • Facilitate educational, psychoeducational, and recovery-oriented support groups.
  • Provide workshops focused on wellness, coping skills, relapse education, independent living skills, housing retention, and community integration.
  • Encourage client participation in program activities, external recovery resources, and independent living planning.
  • Maintain accurate, timely, and comprehensive case records in CARES and the client’s chart.
  • Complete progress notes, intake forms, service plans, referrals, incident reports, and discharge summaries in accordance with established procedures and relevant regulatory requirements.
  • Participate in case conferences, administrative reviews, program audits, and quality improvement initiatives.
  • Ensure absolute confidentiality and compliance with HIPAA and all applicable agency regulations.
  • Actively participate in interdisciplinary team meetings, administrative supervision sessions with the Team Leader, training, and staff development activities.
  • Provide operational consultation and support to other program staff regarding client behavioral and environmental needs.
  • Assist with program development, implementation, and evaluation activities.
  • Contribute to the maintenance of a safe, respectful, and recovery-oriented residential environment.
