Job summary

Wellstar Health System in Georgia is seeking a Care Coordinator RN to assess transitional care needs and coordinate care across the continuum. You will engage with patients and families to ensure discharge planning, optimize resources, and support care progression.

The RN is expected to perform psychosocial assessments, discharge planning, and collaborate with physicians, UM and care teams to facilitate timely transitions and maintain patient-centered care.

Qualifications

• 1 year nursing experience in the acute care setting is required.
• Active RN license and BLS certification required.
• Excellent communication and teamwork skills.

Responsibilities

• Assess patients' transitional care needs and plan discharge.
• Coordinate with PAS, UM, physicians to ensure timely progression.
• Lead patient/family education and care conferences.
• Document care plans and track discharge barriers.
