Wellstar Health System is seeking a Care Coordinator RN to assess transitional care needs and coordinate care across the continuum. You will engage with patients and families to ensure discharge planning and resource utilization align with goals.

Qualifikationen

• Excellent written and verbal communication skills are required.
• Strong assessment, interview, organizational and problem-solving skills.
• Knowledge of local, state and federal regulations.
• Ability to work collaboratively with physicians, care team and patient/family to progress care through discharge.

• Initiates and documents discharge planning for assigned patients.
• Collaborates with care team to support care progression and discharge plans.
• Facilitates patient/family education and engagement throughout transition.
• Participates in interdisciplinary rounds to optimize discharge timing.
