Job summary

Responsibilities

• Assess patient psychosocial and discharge needs.
• Coordinate discharge planning with the care team and PAS/cinical partners.
• Collaborate with physicians to support care progression and discharge plans.
• Document assessments and care plans in the medical record.
• Identify and address barriers to timely discharge.
• Participate in interdisciplinary rounds and coordinate post-acute referrals.
