Responsibilities

• Initiates assessment of chronic disease management and psychosocial needs.
• Manages discharge planning and coordinates post-acute care transitions.
• Facilitates care progression and documents interactions in the medical record.
• Collaborates with physicians, care team, and social work to resolve barriers.
