Qualifications

• Associates degree in nursing or diploma; bachelor’s preferred.
• RN license (single state) or RN-COMPACT license.
• BLS certification required.
• Minimum 1 year nursing experience in acute care.
• Excellent written and verbal communication skills.
• Knowledge of local, state and federal regulations.
• Ability to collaborate with physicians and care team for care progression.

Responsibilities

• Assess patients' chronic disease management needs and psychosocial risk factors.
• Coordinate discharge planning and ensure timely progression of care.
• Collaborate with care team and physicians for discharge plans.
• Document assessments and progress in the medical record.
• Identify barriers to discharge and escalate appropriately.
• Participate in interdisciplinary rounds and post-acute referrals.
