Job summary

Qualifications

• Associates degree or nursing diploma required; BSN preferred.
• RN license valid for the state or compact multi-state license.
• BL S certification required.
• Minimum 1 year of acute care nursing experience.

Responsibilities

• Assess patients' chronic disease management needs and psychosocial risk factors.
• Coordinate discharge plans with care team and family to ensure timely progression.
• Collaborate with physicians, PAS, UM, and social work to resolve barriers.
• Document care activities clearly in the electronic medical record.
