Job summary

Wellstar Health System seeks a Care Coordinator RN to assess transitional care needs and coordinate care across the continuum. You will engage with patients and families to ensure discharge planning aligns with medical goals and resource availability.

Collaborates with physicians, UM and care team to optimize progression of care, address barriers, and document care plans. Requires strong communication and organizational skills for fast-paced environments.

Responsibilities

• Assess patients' chronic disease management needs and psychosocial risk factors.
• Coordinate discharge planning and communicate with physicians and care team.
• Collaborate with PAS, financial counselor, and UM nurse to verify payor requirements.
• Participate in interdisciplinary rounds and care conferences.
• Identify and address barriers to timely discharge.
• Document care progression and ensure up-to-date records.
• Refer post-acute cases for social work when needed.
• Serve as preceptor for interns (if appropriate).
