Job summary

Responsibilities

• Assessment of chronic disease management needs and psychosocial risk factors.
• Discharge planning and collaboration with financial and UM teams.
• Coordinate care progression and communicate with physicians and care teams.
• Document care plans and progress in medical records, track outcomes.
• Participate in interdisciplinary rounds and post-acute referrals.
• Perform other duties as assigned.
