Fred Hutchinson Cancer Center in Seattle is hiring a Transition Registered Nurse for 0.95 FTE (38 hours per week). The role focuses on transition and discharge planning, care coordination with a multi-disciplinary team, and education for patients and families across the continuum of care.

Oncology experience is valued; relocation assistance may be available. The position offers comprehensive benefits and requires WA RN licensure with ONS chemotherapy training within 3 months.

Competenze

• Graduate from an accredited school of nursing.
• Current WA State RN license or MS license with ability to transfer within 60 days.
• Oncology experience preferred; training provided.
• BLS certification required with renewal every 2 years.
• ONS Chemotherapy Provider Card within 3 months of hire.
• Excellent teaching and communication skills.

Mansioni

• Assess patients' physical, emotional, social, and spiritual needs.
• Develop and update care plans based on disease and response to treatment.
• Educate patients, families, and caregivers on disease processes and treatment side effects.
• Coordinate with interdisciplinary team to ensure continuity of care.
• Collaborate with home infusion/homecare agencies and SNFs as needed.
• Document assessments and plans in the medical record after each encounter.
• Teach classes for patients and families in 1-on-1 and group settings.
• Serve as liaison with insurance case managers throughout treatment.
