Job summary

Fred Hutchinson Cancer Center in Seattle seeks a Registered Nurse for the Transplant Clinic Transition Team. This 0.95 FTE, 38 hours/week role focuses on transition planning, discharge education, and coordinating care across the continuum for oncology patients.

You will work with a multidisciplinary team, support patient-family education, and ensure high-quality, compassionate care during transitions, with opportunities for training and professional growth in a leading cancer center.

Qualifications

• Graduate from an accredited school of nursing.
• Current Washington State RN license (MSL) or eligible to upgrade to WA within 60 days of hire.
• This may be waived only under specific circumstances with approval.
• Current BLS certification (AHA Health Provider) renewed every two years.
• ONS Chemotherapy Provider Card required within 3 months of hire.
• Strong clinical judgment and time-management skills.
• Excellent communication skills with patients, families and staff.

Responsibilities

• Assesses physical, emotional, social, and spiritual needs and evaluates patients’ adaptation to health changes.
• Develops a plan of care based on disease, symptoms and response to treatment.
• Educates patients, families, and caregivers on disease processes, treatment, side effects and symptom management.
• Coordinates with inter-disciplinary team to ensure continuity of care.
• Refers patients and families to social work, home infusion/home health, nutrition, etc.
• Implements, evaluates and modifies plan of care.
• Integrates research and evidence-based knowledge into clinical practice.
• Provide Transition Planning at UW Medical Center and Fred Hutch Clinic.
• Participate in assessments and evaluations of patient/family care throughout transitions.
• Coordinate patient care plans with home infusion/home care and skilled facilities.
• Document assessments and plans in Medical Record after each encounter.
• Collaborate with multi-disciplinary team to ensure continuity of care.
• Teach classes for patients and families 1-on-1 and in classrooms.
• Serve as liaison with insurance case managers during treatment process.
• Communicate respectfully with staff and act as Clinic ambassador to inpatient units and referrers.
• Facilitate as smooth a transition as possible and support patient-family centered care.
• Listen to concerns of patients/families objectively.
• Review complex discharges with Transition Supervisor to align messaging.
