Job summary

Fred Hutchinson Cancer Center in Seattle seeks a Transition Registered Nurse to lead discharge planning and care coordination across the continuum. You will assess patients, plan care, educate families, and collaborate with home infusion and SNF teams to ensure seamless transitions.

This 0.95 FTE role requires RN licensure, BLS, and chemotherapy knowledge; oncology experience is a plus. The center offers a comprehensive benefits package and relocation assistance where applicable.

Qualifications

• Graduate from an accredited school of nursing.
• Current Washington State RN license (MSL) or ability to obtain WA MSL within 60 days.
• Oncology experience preferred; training provided if needed.
• Current BLS certification; ONS Chemotherapy Provider Card within 3 months of hire.

Responsibilities

• Assess patients’ physical, emotional, and social needs and evaluate adaptation to health changes.
• Develop care plans based on disease, symptoms and response to treatment.
• Educate patients, families, and caregivers on disease processes, treatment, and symptom management.
