Callen-Lorde Community Health Center in Manhattan, NY is seeking a Doula Surgery Integrated Health Navigator to assist TGNB patients in navigating medical and supportive services for recovery care. The role emphasizes culturally competent outreach and collaboration with community organizations.

Responsibilities include assessments, care planning, and developing TGNB-focused health education materials while engaging diverse populations and working within a multidisciplinary team.

Qualifications

• High school diploma or GED required.
• Direct TGNB service experience or relevant TGNB life experience.
• Experience in social services, case management and/or entitlements work with TGNB, LGB, and HIV-affected communities.
• In-depth knowledge of TGNB health issues and NY state TGNB health insurance requirements.

Responsibilities

• Perform assessments and develop a personalized care plan.
• Develop relationships with community organizations supporting TGNB communities.
• Conduct outreach activities including community visits and consults.
• Follow up appointments.
• Assist in developing health education materials for TGNB health needs.
