Job summary

Callen-Lorde Community Health Center seeks a Doula Surgery Integrated Health Navigator to assist TGNB patients in navigating recovery care, with assessments and personalized care plans.

You will build relationships with community organizations, conduct outreach, and contribute to health education materials addressing TGNB health needs, while maintaining strong computer skills and attention to detail.

Qualifications

• High school diploma or GED preferred.
• Direct TGNB service experience or relevant TGNB life experience.
• Experience in social services, case management and/or entitlements work preferred; specifically with TGNB, LGB, and HIV-affected communities.
• In-depth knowledge of health and social issues facing the TGNB community and familiarity with service and resources available.
• Knowledge of transgender surgical procedures, WPATH standards of care and NY state TGNB health insurance requirements preferred.

Responsibilities

• Perform assessments and develop a personalized care plan.
• Develop relationships with community organizations supporting TGNB communities.
• Conduct outreach activities to include community visits and consults.
• Follow up appointments.
• Assist in the development of health education materials focused on the health needs and concerns of TGNB communities.
