Qualifications

• Two years office and/or other related experience preferred.

Responsibilities

• Assist the Care Manager in providing services for the caseload and locating patients assigned to the team.
• Update roster information and link patients to care.
• Educate patients on health conditions and provide appointment reminders.
• Maintain case records, scan documents into the EMR, and handle basic office administration.
• Schedule and coordinate appointments for the Care Management team.
• Participate in care conferences and track documentation due dates.
