Job summary

Kona Medical Consulting is seeking a Home Health Field Registered Nurse (RN) to provide skilled nursing care to patients in their homes across Texas. The ideal candidate is reliable, organized, and confident in clinical decision-making while helping patients achieve health goals.

Responsibilities include in-home visits, care planning, wound care, and ongoing patient education, with on-call rotation and strong collaboration with physicians and care teams.

Qualifications

• Current RN license in the state of Texas.
• 1+ year of nursing experience (Home Health preferred but not required).
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation, valid driver’s license, and proof of auto insurance.
• Comfortable using EMR/mobile charting.
• Compassionate, professional, and patient-focused.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits
• Develop and update individualized care plans
• Perform wound care, disease management, medication administration, and patient education
• Communicate effectively with physicians, case managers, and interdisciplinary teams
• Complete accurate, timely documentation in accordance with regulatory requirements
• Participate in on-call rotation as needed
• Build trusting relationships with patients and families
