Kona Medical Consulting and The Home Health Team is seeking a Home Health Field Registered Nurse (RN) in Texas to provide skilled nursing care in patients’ homes across multiple areas. You will assess, educate, and support patients while coordinating with physicians and care teams.

The role emphasizes compassionate patient care, reliable mobility, and autonomy in daily workflows, with strong emphasis on documentation and safety.

Qualifications

• Current RN license in Texas is required.
• 1+ year of nursing experience (Home Health preferred).
• Strong clinical assessment and critical-thinking skills are expected.
• Reliable transportation, valid driver’s license, and proof of auto insurance.
• Comfortable using EMR/mobile charting.
• Compassionate, professional, and patient-focused.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Provide wound care, disease management, medication administration, and patient education.
• Communicate effectively with physicians, case managers, and interdisciplinary teams.
• Complete accurate, timely documentation in line with regulatory requirements.
• Participate in on-call rotation as needed.
• Build trusting relationships with patients and families.
