Kona Medical Consulting is seeking a dedicated Home Health Registered Nurse (RN) to perform in-home assessments and provide skilled nursing visits, delivering high-quality care in patients' homes across Texas.

The ideal candidate will develop and update care plans, administer medications, manage wounds, and collaborate with physicians and care teams, while maintaining thorough documentation and contributing to a supportive, patient-centered home health program with ongoing training.

Qualifications

• Current RN license in Texas
• 1+ year of nursing experience (Home Health preferred but not required)
• Strong clinical assessment and critical-thinking skills
• Reliable transportation, valid driver’s license, and proof of auto insurance
• Comfortable using EMR/mobile charting
• Compassionate, professional, and patient-focused

Responsibilities

• Perform in-home patient assessments and skilled nursing visits
• Develop and update individualized care plans
• Perform wound care, disease management, medication administration, and patient education
• Communicate effectively with physicians, case managers, and interdisciplinary teams
• Complete accurate, timely documentation in accordance with regulatory requirements
• Participate in on-call rotation as needed
• Build trusting relationships with patients and families
