Kona Medical Consulting and The Home Health Team is seeking a Home Health Field RN to provide skilled nursing care in patients' homes across Texas. The ideal candidate is reliable, organized, and confident in clinical decision-making to support patient goals.

You will perform in-home assessments, develop care plans, manage wounds and medications, educate patients and families, and document thoroughly. Requires Texas RN license and 1+ year of nursing experience; EMR experience is a plus.

Qualifications

• Current Texas RN license.
• 1+ year nursing experience; Home Health preferred.
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation, valid driver’s license, and proof of auto insurance.
• Comfortable using EMR/mobile charting.
• Compassionate, professional, and patient-focused.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Perform wound care, disease management, medication administration, and patient education.
• Communicate effectively with physicians, case managers, and interdisciplinary teams.
• Complete accurate, timely documentation in accordance with regulatory requirements.
• Participate in on-call rotation as needed.
• Build trusting relationships with patients and families.
