Job summary

Kona Medical Consulting and The Home Health Team is seeking a Home Health Field Registered Nurse (RN) to provide skilled nursing care to patients in their homes across Texas. You will perform in-home assessments, develop care plans, manage wounds, and educate patients, working with physicians and care teams.

The ideal candidate has an active Texas RN license, 1+ year of nursing experience (home health preferred), strong assessment skills, reliable transportation, and comfort with EMR/mobile

Qualifications

• Current RN license in Texas.
• 1+ year of nursing experience (Home Health preferred but not required).
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation, valid driver’s license, and proof of auto insurance.
• Comfortable using EMR/mobile charting.
• Compassionate, professional, and patient-focused.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Perform wound care, disease management, medication administration, and patient education.
• Communicate effectively with physicians, case managers, and interdisciplinary teams.
• Complete accurate, timely documentation in accordance with regulatory requirements.
• Participate in on-call rotation as needed.
• Build trusting relationships with patients and families.
